The eBaby service is once again available on the Diia portal, allowing users to quickly submit up to 9 state services with a single application.
In this regard, the Odessa City Council's Center for Administrative Services is also resuming the acceptance of documents for the eBaby electronic public service.
Parents can apply for the eBaby service if:
- the child does not yet have a birth certificate;
- the child was born in a medical facility in Ukraine;
- both parents are citizens of Ukraine.
Required documents to submit an application:
- marriage certificate (if available),
- passports of both parents as Ukrainian citizens,
- TIN (Tax Identification Number) of both parents,
- unique registration number from the Unified State Demographic Register for at least one parent,
- electronic medical certificate of birth or medical birth certificate (form No. 103/o),
- certificate of refusal to receive the one-time natural assistance "Baby Package" from the maternity hospital,
- bank social account number of the child benefit recipient (the parent on parental leave),
- "PrivatBank" card,
- special account for the "Baby Package" (if refusing the natural assistance "Baby Package").
Newborn parents need to make an appointment in the electronic queue to receive the comprehensive service.
